A copy of a valid photo ID must be included if the form is mailed, uploaded, or faxed. If you are picking up records in person, you will be required to present a valid photo ID at the time of pickup.
Completed forms may be mailed to Student Health, 6100 Main Street, Houston, TX 77005, or faxed to 713-348-5427. Please call us at 713-348-4966 to discuss your request prior to submission.
As a courtesy to Rice students and alumni, we do not charge a fee for medical records requests.
