Information on Student Health Insurance, the Health Data Form, Frequently Asked Questions related to the Health Data Form, Meningococcal Vaccines, and the Parent Consent Form for Minors, is included on the subsequent pages. Each page includes the most up-to-date information for each topic. All students must turn in a Health Data form which requires a physical exam by a medical provider.
DUE JUNE 15 for UNDERGRADS, JULY 1 for MBA and JULY 15 for GRADUATE STUDENTS.
A visit with your home physician will be needed to finish the form. You may see a provider in your home country.
